New Zealand Licence
The New Zealand SNOMED CT® Release Centre will be distributing the current version of SNOMED CT® via CD-ROM. To receive a copy of the latest version of SNOMED CT® you will need to read and agree to the Licence Agreements and complete the online the Application Form.
Process for receiving SNOMED CT® content
- Select the links below to download the two Licence Agreements.
- SNOMED CT® Affiliate Licence Agreement (PDF, 112 KB)
- New Zealand National SNOMED CT® Licence Agreement 3.1(PDF, 68 KB)
- Open and read the two the License Agreements documents.
- Read the Licence agreements carefully. Submit any questions using the email address below.
- Click here to fill out the online SNOMED CT® Application Form
- Incomplete applications cannot be accepted and will require re-submission
If, for any reason you are unable to complete the online Application Form, please download the SNOMED CT® Application Form below:
- New Zealand SNOMED CT® Application Form (PDF, 68 KB)
- Fill out the printed copy of the Application Form completely and legibly. Incomplete applications cannot be accepted and will require re-submission.
- Sign the Application Form.
- Send the original signed copy of the Application Form by postal mail to:
