To obtain a HealthSecure digital certificate for authentication purposes, download the following forms as appropriate:
- Organisation Registration Form (a 76 kB Word file or a 128 kB PDF file)
This form is to register an organisation with the Registration Authority so that applications for digital certificates can then be sent. - Organisation Member Form (a 80 kB Word file or a 112 kB PDF file)
This is completed by the person within the registered organisation who is requiring a certificate. - Member Approval List (a 52 kB Word file or a 68 kB PDF file)
This form is to be signed off by an authorised signatory as noted on the Organisation Registration Form. - Server Registration Form (a 48 kB Word file or a 112 kB PDF file)
This form is to request a server certificate for an organisation (previously registered with NZHSRA). The person completing the form will be the person who is responsible for the certificate. - Change of Details Form (a 76 kB Word file or a 100 kB PDF file )
This form is to allow certificate holders and organisations to change details within their registrations. - Revocation Request (a 44 kB Word fileor a 88 kB PDF file)
This form is to allow certificate holders, organisations and other authorised bodies, to request revocation of a certificate.
